REQUESTED ACTION:
It is requested that the Dekalb County Board of Education (“the Board”) approve
and ratify Change Order No. 1 to Evergreen Construction for a not-to-exceed
amount of $468,393 for Owner requested items as outlined in the Change Order
No. 1 documents.


RATIONALE:
Approval and Ratification of Change Order No. 1 will provide necessary funds for
unforeseen conditions with the existing facility, design development and
Owner -DeKalb County School District- requested items as outlined in the
Change Order No. 1 documents for the Tucker Middle School HVAC and
Controls Replacement.

Approval of Change Order No. 1 meets Strategic Goal Area 6:
Organizational Excellence

DETAILS:
On September 11, 2023, the Board of Education approved a firm fixed price contract
to Evergreen Construction, to perform the general contracting work as outlined by
the proposal documents.

This Change Order No. 01 covers the approved additional scope for the Owner -
DeKalb County School District- requested items.

Specific details related to the original scope of work for this Request for Proposal
can be found on the DeKalb County School District’s solicitation website at

SUMMARY:
The Change Order No. 1 becomes effective upon Board approval and fully
executed change order documents.

Specific details related to the changed scope of the project requested can viewed
in the Construction Contract Change Order document and the Supporting
Documents to the change order - Potential Change Order (PCO) Request.

The approval of Change Order No. 1 in the amount of $468,393.00 will provide for
the additional scope of work outlined above by Evergreen Construction for Tucker
Middle School.

The approval of Change Order No. 1 – Design Fee meets COGNIA Standard 5:
Using Results for Continuous Improvement


FINANCIAL IMPACT:
The total amount of $468,3939 for Evergreen Construction Change Order No.
1 will be paid from the General Contractor cost code. A budget reallocation of
$468,393 from FUND 300 to the General Contractor cost code is required to fund
this Change Order No. 1.
