Request: It is requested that the DeKalb County Board of Education (“the Board”) approve Ratification / Change Order No. 4 to Centennial Contractors Inc. for the Henderson Mill Elementary School Major System Replacements project to install a new Security Vestibule in the amount of $332,193.58.
Why: Approval of Change Order No. 4 will provide additional funds to install a new Security Vestibule for the Henderson Mill Elementary School Major System Replacements in the amount of $332,193.58.
Approval of Change Order No. 4 meets Strategic Goal Area 6: Organizational Excellence.
Details: On October 16, 2023, the Board approved the award of General Contracting Services for Major Building System Replacement to Centennial Contractors Enterprises, Inc. in the not to exceed amount of $11,565,900.
This Change Order No. 04 covers the approved additional scope for the DeKalb County School District requested items to Henderson Mill Elementary School Major System Replacement project for the construction of the new Security Vestibule.

Financial impact: The financial impact of the Change Order No. 4 for the additional scope of work for the Henderson Mill Elementary School Major System Replacement project for the construction of the new Security Vestibule will be funded from the Contractor’s cost code SP5FACCON.35135.CONTRACTOR under the voter-approved E-SPLOST V program.

Henderson Mill Elementary School Major Systems Replacement

Scope:
Approval of Change Order No. 4 for expanded scope of work for Owner requested
items to Centennial Contractors Enterprises, Inc. for the Henderson Mill
Elementary School Major Systems Replacement. The Change Order No. 4 is in
the amount of $332,193.58.

Background:
After the May 24, 2016, approval of the 2017-2022 E-SPLOST V referendum by
DeKalb voters, the Board approved an E-SPLOST V list of projects on December 5,
2016, for $561 million to include various capital renewal, renovations, safety and
security upgrades and classroom addition projects throughout the district. Briar Vista
Elementary School and Henderson Mill Elementary School were included in the
Major Building System Replacement project list.
A formal Request for Proposals (RFP) was issued on June 8, 2023, for the Briar Vista
Elementary School and Henderson Mill Elementary School Project. On October
16, 2023, the Board of Education approved the award of the project to Centennial
Contractors Enterprises, Inc.
The approval of Change Order No. 4 is for Centennial Contractors Inc. to perform the
general contracting work for the Dekalb County School District requested items as
outlined by the proposal documents.
This agenda item requests a change order in the amount of $332,193.58
for a total project construction budget of $8,039,546.42 for Henderson Mill
Elementary School Major Building Systems Replacement.
Approval of Change Order No. 04 will provide the staff and students at Henderson
Mill Elementary School with a safe and ideal environment for teaching and
learning.
