Request: It is requested that the DeKalb County Board of Education approve the contract renewal (and ratification) with Caduceus USA Medical Services, LLC for an additional year through June 30, 2025, for a not to exceed amount of $200,000 for occupational medical services. This is the fourth and final (#4 of 4) one (1) year renewal option.
Why: Renewal of the contract with Caduceus USA Medical Services, LLC will ensure compliance with Federal Motor Carrier Safety Administration's (FMCSA) regulation of Drug and Alcohol Testing 49 CFR 382 and the Georgia Department of Education's physical requirements. This ensures that safety-sensitive positions such as DCSD school bus drivers are in compliance with State and Federal regulations.
Approval of the contract renewal meets Strategic Goal Area 6: Organizational Excellence
Details: On June 8, 2020, the Board of Education approved the contract award with Caduceus USA Medical Services, LLC to provide occupational medical services through one vendor. Previously, the services were provided by three (3) separate vendors.
This is the fourth and final (#4 of 4) one (1) year renewal option remaining on the contract.
Financial impact: Occupational medical services will be charged from charge code 100.2700.530000.00011.7100.1320.8012.040.0000, for a not to exceed amount of $200,000.

Scope: Provision of Occupational Medical Services – Drug and Alcohol testing, physical
examinations, and Medical Review Officer for Administration of FMCSA standards.
Contract Term:
On June 8, 2020, the Dekalb Board of Education approved the award of the contract for
an initial one (1) year term for a not to exceed amount of $200,000 with four (4) one (1)
year options to renew subject to Board approval on a year-to-year basis.
The renewal terms are as follows:
• Contract Year 2, Renewal #1 of 4 – July 1, 2021 – June 30, 2022, Not to Exceed
$200,000.
• Contract Year 3, Renewal #2 of 4 – July 1, 2022 – June 30, 2023, Not to Exceed
$200,000.
• Contract Year 4, Renewal #3 of 4 – July 1, 2023 – June 30, 2024, Not to Exceed
$200,000
• Contract Year 5, Renewal #4 - July 1, 2024 – June 30, 2025, Not to Exceed

Solicitation Process:
Pursuant to Board Policy DJE Section III, C (3) (4), the choice of medical service
provider was based a full assessment of professional abilities and other relevant criteria
versus a competitive bidding process. Additionally, Board Policy DJE Section II,
includes medical services in the professional services category which is typically better
suited to a qualitative or subjective method of assessment rather than a quantitative or
objective method of assessment.

Award Process:
Caduceus USA Medical Services was selected based on the above policy with
major considerations given to criteria such as driver access to vendor facilities in Dekalb
county, vendor capacity to process a high volume of physicals, drug and alcohol tests,
DOT compliant testing services and competitive pricing.
