i. Contract Renewal and Ratification- Medical Review Officer Services- Caduceus USA Medical Services, LLC - Contract Renewal #4 of 4 (Not to Exceed $200,000)

Request: It is requested that the DeKalb County Board of Education approve the contract renewal (and ratification) with Caduceus USA Medical Services, LLC for an additional year through June 30, 2025, for a not to exceed amount of $200,000 for occupational medical services. This is the fourth and final (#4 of 4) one (1) year renewal option.
Why: Renewal of the contract with Caduceus USA Medical Services, LLC will ensure compliance with Federal Motor Carrier Safety Administration's (FMCSA) regulation of Drug and Alcohol Testing 49 CFR 382 and the Georgia Department of Education's physical requirements. This ensures that safety-sensitive positions such as DCSD school bus drivers are in compliance with State and Federal regulations.
Approval of the contract renewal meets Strategic Goal Area 6: Organizational Excellence
Details: On June 8, 2020, the Board of Education approved the contract award with Caduceus USA Medical Services, LLC to provide occupational medical services through one vendor. Previously, the services were provided by three (3) separate vendors.
This is the fourth and final (#4 of 4) one (1) year renewal option remaining on the contract.
Financial impact: Occupational medical services will be charged from charge code 100.2700.530000.00011.7100.1320.8012.040.0000, for a not to exceed amount of $200,000.

Reference: Medical Review Officer Services – Professional Service Agreement
Greetings:
As a result of the excellent service provided by Caduceus US Medical Services, LLC, the DeKalb County School District (“DCSD”)
desires to renew the professional service agreement of Medical Review Officer Services for one (1) year on the same terms,
conditions as set forth in the License and Services Agreement between DCSD and Caduceus US Medical Services, LLC. The
renewal term will utilize the proposed price increase as indicated in your letter dated August 3, 2024. The purpose of this letter
is to obtain Caduceus US Medical Services, LLC’s acceptance of DCSD’s offer to renew the License and Services Agreement.
The renewal is subject to the DeKalb County Board of Education’s (“Board”) approval and will be effective through June 30,
2025. Of course, we will notify you once the Board has approved the renewal. DCSD Caduceus US Medical Services, LLC’s
consideration of this offer to renew the professional service agreement.
If accepted, please submit a copy of your company’s proof of insurance reflecting the coverage(s) stated within the original
solicitation document, sign the acceptance below and email both documents no later than Monday, July 29, 2024 to
sharmaine_greenland@dekalbschoolsga.org. Insurance policy or policies must be maintained throughout the term of this
agreement. A copy of the insurance requirements is included.

ACKNOWLEDGMENT
Caduceus US Medical Services, LLC hereby accepts DeKalb County School District’s offer to renew the professional service
agreement as set forth in the License and Services Agreement, until June 30, 2025. Caduceus US Medical Services, LLC
understands that this acceptance is subject to the approval of the DeKalb County Board of Education.
