I am pleased to inform you that our Board of Directors has approved a grant of $50,000.00
to DeKalb High School Technology South to be used for the Building Future Investigators and
Fabricators Through STEAM project, as described in your grant application which was received
on 05/06/2026.

Enclosed please find the following documents needing your attention:

   1. Letter of Agreement – Please sign
   2. ACH Organization Authorization Form – Please fill in all information and include any
      additional documents required so that we may process your grant payment via ACH

Please note that we also require your most recent W-9: Form W-9 (Rev. October 2018) (irs.gov).

Once you have signed/completed the enclosed documents, please scan and upload them to
the Dart Foundation grantee portal. Upon receipt of your signed letter of agreement,
completed ACH form, and your organization’s W-9, the Dart Foundation will generate an ACH
payment for $50,000.00.

We ask that you complete any Grant Progress Report forms that are assigned to you on our
grantee portal by their requested due dates. We also ask, if feasible, that you forward a
few pictures of the “grant/program” in action for possible inclusion on our website
and/or in future foundation publications.

Dear Dr. Allen:

The Dart Foundation ("Foundation") is pleased to inform you that its Board of Directors
(“Board”) has approved a grant of $50,000.00 to Grantee to be used for the Building Future
Investigators and Fabricators Through STEAM project, as described in your grant application
which was received on 05/06/2026. Because you are recognized as a federally tax-qualified
organization under Sections 501(c)(3), 509(a), 170(b) of the Internal Revenue Code or
otherwise hold federal tax-exempt status, we kindly request that you sign and return this letter
of agreement to assure compliance with the Internal Revenue Code.

(1) All of the grant funds are to be used for, and only for, the Building Future Investigators and
Fabricators Through STEAM project. Use of any portion of the grant funds, including any
interest thereon, for any other purpose must have the prior written approval of the Foundation.
You agree to notify the Foundation of any material changes to the project/organization such as
changes to financial stability, key personnel, or the scope and nature of the project. The
Foundation reserves the right to alter or modify this grant in the event of a material change.

(2) Grantee agrees to furnish annual, written reports to the Dart Foundation on the manner in
which the grant funds were spent and the progress made in accomplishing the grant's purposes.

(3) Grantee agrees that the grant funds will be used in accordance with the budget submitted
by Grantee as part of its grant application. Grantee agrees to furnish periodic reports to the
Dart Foundation as to the expenditure of the grant funds in accordance with the budget and a
final report upon completion of the grant period.

(4) Grantee agrees to keep its financial and other records so that they adequately show the
use of the grant funds exclusively for the grant's purposes.

(5) Grantee assures the Foundation that the grant will not cause Grantee to lose its federally
tax-qualified status as described under Sections 501(c)(3), 509(a), 170(b) of the Internal
Revenue Code or otherwise held federal tax-exempt status and that its determination letter(s)
from the Internal Revenue Service is (are) still valid and has (have) not been revoked.

(6) Grantee agrees to supply the Foundation with such other information as may be necessary
or desirable to permit the Foundation to review the use made of this grant and its effect upon
the status of Grantee.

(7) Grantee agrees that the Foundation will have the right to approve, restrict and/or edit any
communication regarding this grant prior to the release or dissemination of information by
Grantee to a third party regarding this contribution.

(8) Grantee agrees to work with the Foundation to develop mutually agreeable recognition and
publicity opportunities for this grant including but not limited to any signage and/or press
opportunities. More specifically, Grantee agrees to allow Foundation to have tasteful signage or
its name listed on a donor wall, commensurate with other comparable donors. The Foundation
shall approve any signage in writing before it is affixed in any space.

(9) Grantee authorizes the Foundation to use any photos, testimonials, or other content the
Grantee provides to the Foundation and agrees that it has obtained any necessary approvals
from any of the individuals referenced in or appearing in such content.

(10) Grantee agrees not to use the Foundation’s name or logo without the Foundation’s prior
permission.


Signatory, on behalf of the Grantee, hereby agrees to the terms and conditions of the grant as
recited above and warrants that they are authorized to bind the Grantee to the obligations set
forth herein.

                                 ACH Authorization Form

______________________(“ORGANIZATION”) has been approved for a grant or vendor payment from
the Dart Foundation (“FOUNDATION”). FOUNDATION desires the flexibility to make payments
electronically through the Automated Clearing House (“ACH”) Network. ORGANIZATION agrees to
receive payments for such grants, products and/or services purchased by FOUNDATION through ACH.

Therefore, ORGANIZATION hereby (1) authorizes FOUNDATION to deposit payment by ACH directly into
the account at the depository financial institution selected by ORGANIZATION, listed below, (2)
authorizes FOUNDATION to correct any errors which may occur from these transactions, and (3) directs
that all such deposits be made as provided below:

notice from ORGANIZATION, of any changes in the depository financial institution or other payment
instructions listed above.

   •  One (1) signature and a voided check; or,
   •  One (1) signature with a letter from a banking institution confirming banking
      information

This Authorization will become effective within seven (7) days after its receipt by the FOUNDATION.
